Service Eligibility

  • If you live within our local home visit service area (ZIP codes below): you’ll be guided to book a covered home visit.

  • If you live outside the local home visit service area: your Wildflower plan covers virtual visits, so you’ll be guided to schedule a telehealth consultation.

  • Home visits are available Monday–Thursday, and virtual visits are scheduled on Fridays.
